Diagnosis begins with a clinical examination: the surgeon palpates the chest to distinguish firm glandular tissue (true gynecomastia) from soft fatty enlargement (pseudogynecomastia), grades the severity from I to IV, and checks skin quality. A careful history covers medications, supplements, anabolic steroid use, alcohol intake and pubertal timeline.
Supporting tests may include an ultrasound of the chest to map gland versus fat, and blood tests - liver and kidney function, thyroid profile and hormone levels - to rule out an underlying cause. In rare cases of one-sided, hard or rapidly growing lumps, further imaging is done to exclude male breast cancer before planning cosmetic surgery.
